Kelly repair involves radical mobilization of the periosteum of the pubis and ischium together with the attachments of all sphincter and levator ani muscles, the pudendal nerves and vessels, and the corpora cavernosa without any form of pelvic osteotomy.
This procedure allows an innervated total sphincter wrap around the new pelvic urethra without tension.
